I am an out of network provider and do not file insurance claims. However, I am happy to provide superbills upon request so that you can submit to your insurance company for potential reimbursement. Superbills include diagnostic codes and all the information you need to self-file for services rendered.
No. You do not need a referral for Physical Therapy in the state of North Carolina. However, I believe that communication with your overseeing physician is often key for comprehensive health management. I am more than happy to discuss your care with them during the course of your treatment.

“Rachael gave me back my dignity and my mobility. Others said I should be happy with 80% range of motion but her knowledge and mix of modalities with PT has made all the difference."
-Carol
"I am more than excited to remain a part of Rachael’s physical therapy treatments! She and I go back to 2017 and she’s seen me through a Variety of orthopedic issues and maladies. She is such a creative therapist, but has the professional experience that always makes a way for me to feel better while functioning to my best ability. One more thing that I found in Rachael—she has such a heart for what I’m dealing with. She “gets” me and there’s always a little bit more in her tank for me!"

"Rachael Shoffner has been my PT providing physical therapy for lymphedema in my right arm and shoulder after several surgeries. Her knowledge and exceptional care to relieve my pain and increase mobility as well as advice on daily living skills has improved my quality of life and has been extremely helpful."
-Dorothy K.
"Rachel Shoffner is a skillful and intelligent practitioner. Her ability to assess and provide a variety of techniques and exercises has enabled me to gain great range of motion and use of my dominant arm and shoulder after chest surgeries. Just as important, she worked to develop a honest and trusting relationship to empower me to exercise at home. I gained confidence in my physical abilities to move forward in my life after a cancer diagnosis. I see her as a wonderful professional, an important part of my "care" team and friend."
-Susan S.
"I am delighted to share my experience as a patient of Rachael Shoffner, Physical Therapist and owner of PT Resilience. As a two-time metastatic breast cancer survivor, finding a healthcare professional who not only possesses extensive knowledge but also genuinely cares about my well-being has been instrumental in my journey towards a better quality of life."

"Rachael – I’ll just put down several thoughts. You can use it as a whole or break it into segments as you see fit. You can cut and paste it into various pieces. I’ll soon turn 70 and am working my way toward retiring from working 12-hour nursing shifts and am currently down to one 12-hour shift a week. I had hopes that walking several miles a day in my nursing shift would help me be strong, but I was having several issues. And so, I wanted to take time to work on myself to get stronger, more flexible, and improve several aspects of my body."
